Your Rights
• Patients have a right to book an appointment with whichever health professional they choose if available.
• Patients will be treated as individuals as partners in their healthcare, irrespective of their ethnic origin or religious and cultural beliefs.
• We will give you full information about the services we offer. Every effort will be made to ensure that you receive the information which directly affects your health and the care being offered.
• You have the right to see your health records, subject to limitations in the law.

• Please do everything you can to keep appointments; if you cannot attend please inform the surgery so your appointment can be released to another patient.
• Let us know if you change your name, address or telephone number.
• We ask that you treat the doctors and practice staff with courtesy and respect.
• Remember, you are responsible for your own health and the health of your children. We will give you our professional help and advice. Please act upon it.
Practice Complaints Procedure
If you have a complaint or concern about the service that you have received from the doctors or any of the staff working in this practice, please let us know. We operate a practice complaints procedure as part of an NHS system for dealing with complaints. Complaints should be addressed to the practice manager. A practice complaints leaflet is available from reception.
Confidentiality
The practice recognises that patient confidentiality is paramount and fully
adheres to the Data Protection Act 1998. We respect your right to privacy and
keep all your health information confidential and secure. It is important that
the NHS keeps accurate and up-to date records about your health and treatment
so that those treating you can give you the best possible advice and care. This
information is only available to those involved in your care and you should never
be asked for personal medical information by anyone not involved in your care.
You have a right to know what information we hold about you. If you would like
to see your records, please write to the practice manager.
Freedom
Of Information - Publication Scheme
The Freedom of Information
Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme
is a guide to the classes of information the practice intends to routinely make
available. The scheme is available from reception.
Violent or Abusive Patients
The practice will not tolerate violent or abusive patients and fully subscribes to the NHS Zero Tolerance Policy which means removal of the patient from the practice list and the patient being allocated a specific practice by the Primary Care Trust. The practice will not tolerate swearing or abusive behaviour via the telephone or in person. If this should happen the conversation will be terminated and the patient will not be dealt with.
